Receiving Continued Care for Chronic Addiction
Because JourneyPure recognizes that addiction is a chronic condition, our commitment to your health and recovery doesn’t end when you leave your treatment stay. A strong continuous care plan is key to maintaining long-term recovery from the chronic conditions of drug addiction and alcoholism. Once a residential inpatient program is completed, there may be step-down treatment through intensive outpatient care. JourneyPure’s patients have access to discharge planners to help outline what extended care options are available and what the step down treatment plan should be. Each patients course of treatment will be different, and we tailor our plans to support your ongoing needs. Through recovery coaching in our JourneyPure Coaching app, we are able to maintain contact with patients following discharge even if they are not local to where they received treatment.
Some patients may choose to follow up residential treatment with lower levels of care before discharging. This typically occurs in the form of Intensive Outpatient Care. We will work with each patient to identify what step-down options best suit their needs.
